Joining the Patient Participation Group (PPG)

How do I join?

Thank you for your interest in joining Modality Lewisham’s Patient Participation Group (PPG). By joining the PPG, you will become part of the valued patient voice working with the practice to help improve services for all patients.

Please complete the below online form to register your interest in joining your usual site’s Patient Engagement Forum (PEF), which is part of the wider Patient Participation Group (PPG).

On completion of the online form, your information will be shared with the chair and secretary of the Patient Engagement Forum (PEF) at your usual site in-line with the PPG’s Privacy and Data Policy statement. Your request to join will be reviewed, and the PEF will be in-touch with you directly.
